Housing Preferences
First-year housing requires application submission and all health forms to be received by June 1. Students submitting after June 1 may be placed on a wait list until space becomes available. Hall preference requests are not guaranteed, and assignments are made upon space availability.

Proof/Waiver of Immunization Form (required)
The state of Missouri requires all college students to receive several immunizations before residing on campus.To fulfill this requirement Calvary University requires all students intending to live on campus to complete and submit this form verifying they have received the required immunizations. This form and proof of immunization must be submitted before a room is reserved for any student. If a student has not received all state required immunizations they must complete the proof of exemption portion of this form.
Proof/Waiver of Student Health Insurance Coverage
Calvary University strongly recommends that all students carry health insurance that covers them in the Kansas City area. All students on campus must complete and submit this form along with a copy of their insurance card (or other proof of coverage) prior to their arrival to campus (or before a room is reserved for any student intending to live on campus). If the student is unable or unwilling to obtain health insurance coverage for the Kansas City area there is a place to indicate this at the bottom of this form.

Tuberculosis Health Assessment
All students on campus must complete and submit this form prior to their arrival to campus (or before a room is reserved for any student intending to live on campus). If the student answers yes to any of the questions, they are required to be seen by a public health agency or an appropriate health professional to rule out latent tuberculosis infection or the possibility of developing the tuberculosis disease (proof must be provided).
