If a student has completed a 2013-2014 Free Application for Federal Student Aid (FAFSA) and they were selected for review in a process called verification, there are specific answers that they provided on their FAFSA that Calvary must verify before we can process and award Federal Financial Aid to the student. If you have received a notice from the Financial Aid Office at Calvary that you need to complete and submit documentation, please download the appropriate document(s) from those listed below. Be sure to completely read the entire form and follow all instructions, including attaching all required documents and signing the form as indicated. Correctly completed documentation must be returned to the Financial Aid Office. Please return the forms in person, by emailing email@example.com or by faxing the forms to 816-425-6134. Please note the Financial Aid office at Calvary must receive all correctly completed and signed documents, and all conflicting information must be corrected no later than 30 days before the end of the semester you wish to receive Federal Financial Aid for.
Failure to submit the required documentation correctly completed, may result in the student being unable to receive Federal Financial Aid, increasing the amount the student must pay out of pocket. Until we have received the required documentation to complete the students financial aid file, the Financial Aid Office is unable to offer the student a final financial aid award or disburse any funds.
