Complete Forms

  1. Download the appropriate application from the list to the right.
  2. Complete a My Life With Christ worksheet. This worksheet will guide you in completing your personal testimony. Reenrolling students do not need to submit a worksheet.
  3. Submit a Photo. Your photo will be displayed on a bulletin board so that faculty, staff, and students can put your face with your name and remember to pray for you. Sending a photo is optional.
  4. Mail your application, “My Life with Christ,” and photo, to the following address:

Request Transcripts

  1. High School: If you have not already earned an associate's degree or higher, request your high school transcript, home school transcript,or G.E.D. to be sent to Calvary. If you are currently attending high school, request a partial transcript now and your final transcript after you graduate. If you’ve earned a G.E.D., make a request to have a copy sent to Calvary by visiting www.acenet.edu. Reenrolling students do not need to request a high school transcript.
  2. College: Request college transcripts to be sent to Calvary from all colleges you have attended, whether or not you desire to transfer any credit hours. Reenrolling students should request transcripts from all colleges attended since attending Calvary.

    Transcripts are not considered official unless mailed directly from the issuing institution. 

Request ACT/SAT scores

  1. To request ACT scores, see www.act.org(Calvary’s code: 2312).
  2. To request SAT scores, see www.collegeboard.com (Calvary’s code: 6631).
  3. Have your test scores and transcripts sent to the following address:

    An ACT or SAT score is required for applicants who have not already completed 12 or more semester hours of college. The scores must be mailed to Calvary directly from ACT or SAT, or they must be included on your official high school transcript. 

Contact References

  1. Ask a pastor, elder, or deacon at your church to complete a Pastoral Reference Form. This person should have known you for at least one year and should not be a member of your immediate family. Reenrolling students must submit a pastoral reference if two or more semesters at Calvary have been missed.
  2. Ask a teacher, co-worker, school official, employer, ministry director, etc. to complete a Personal Reference Form. This person should have known you for the past two years and should not be a member of your immediate family. Reenrolling students are not required to submit a personal reference.Fill out the box at the top of each form before giving it to your reference. As a courtesy, please provide the individual with an addressed envelope and postage.

All Done?

You will be notified by mail when your application is received. When all application materials are received, your file will be considered by the Admissions Committee. Under normal circumstances, you will be notified of the decision by mail within two to three weeks after your file is complete.

Upon acceptance, we will ask that you send an enrollment fee of $100 to confirm your decision to study at Calvary Bible College. This fee is non-refundable and must be paid before registering for classes.
